63 year old, 5’2″, 115 lbs, who had previous breast augmentation performed in 1986. She had bilateral ruptured silicone implants placed in a subglandular position. She also had bilateral capsular contracture. She underwent bilateral complete en-bloc capsulectomy with placement of new submuscular 400 cc high profile smooth round silicone implants. Bra size changed from 34B to 34C.

29 year old, 5’4″, 113 lbs, who had previous breast augmentation performed in 2009. She had high profile 550 cc smooth round saline implants. She had apparent significant bottoming out of the right breast and lower pole excessive stretch on both sides. She underwent elevation of the right inframammary crease via capsulorrhaphy, placement of Contour 3 Strattice biologic mesh, and bilateral inferior (crescent) mastopexy. She had placement of 500 cc smooth round moderate plus silicone implants. Her bra size changed from 34DD to 34D.

56 year old, 5’6″, 125 lbs, who had breast augmentation 28 years ago with silicone implants placed in front of the muscle. She had complaints of pain in both breasts with grade 4 capsular contracture on both sides. She underwent en-bloc removal of bilateral ruptures silicone implants and capsules. She then had placement of 400 cc smooth round high profile silicone implants submuscular. The entire procedure was performed through the previous inframammay incisions. Her bra size changed from 34B to 34C.

58 year old, 5’4″, 145 lbs, had breast augmentation performed in 1976. She had placement of silicone implants in front of the muscle. She had a recent mammogram that showed ruptured of bilateral implants as well as severe calcifications. She liked the volume that she currently had and did not want any change in her bra size. She underwent en bloc capsulectomy. 400 cc moderate plus silicone implants were placed in a new location (behind the muscle), through an inframammary incision. Bra size was unchanged, 36C.

66 year old, 5’7″, 135 lbs, had her initial breast augmentation performed in 1978. The implants were placed in front of the muscle (subglandular) at that point. She had bilateral capsular contracture with ruptured silicone implants, that were replaced by another surgeon, in 2011. The implants, moderate plus 275 cc silicone at that point were replaced in the same plane (subglandular) by same surgeon. She shortly after, developed bilateral capsular contracture. She presented to our office with painful breasts. She was obviously very unhappy with the look of her breasts and the multiple surgeries she had to undergo. At this point, we performed a complete capsulectomy and removal of the implants. A new subpectoral plane was established with placement of Strattice Contour 2 biomesh, as well as 450 cc moderate plus silicone implants. Bra size changed from 36C to 36D..

37 year old, 5’5″, 150 lbs, with history of tuberous breast deformity had undergone subglandular augmentation with saline breast implants. She had developed bilateral painful Baker grade IV capsular contracture shortly after her surgery. She underwent bilateral complete capsulectomy, conversion to subpectoral silicone gel implants with a vertical mastopexy. She had placement of 475 cc smooth round high profile silicone implants. Bra size changed from 36c to 36DD.
