As the most feminine feature of a woman’s body, breasts play a major role in both figure and function. As the years pass, they have a tendency to lose their shape, succumbing to the effects of age and gravity. Additionally, many women inherently have breasts they find unsatisfying due to lack of volume. THE BREAST AUGMENTATION PROCEDURE
Once the patient has been properly and safely sedated by our board certified anesthesiologist, Dr. Esmailian will begin the procedure. It starts with an incision, placed in one of the three locations mentioned above. This incision has been fully discussed beforehand so the patient understands the location and shape of the scar as well as the expected recovery time.
With the incision made, Dr. Esmailian creates a pocket for the implant. This pocket will be slightly larger than the implant size. This is because the implant should have some space to move around, which contributes to a natural look and feel. Once the pocket is created, Dr. Esmailian inserts the implant using a device called a Keller Funnel, which minimizes friction and strain between the implant and the incision, lending further safety to the procedure.
At this point, Dr. Esmailian will close the breast tissue leading to the pocket as well as the outer incision. He does this using precise suture techniques to minimize scarring. Once this is done, the breast augmentation is complete.
AFTER BREAST AUGMENTATION SURGERY AT OUR ORANGE COUNTY SURGICAL CENTER
Immediately following your surgery, the first step is to relax in our recovery room. You can remain there until you are fully awake, at which point you will be released to go home. Transportation should be arranged beforehand, as you will not be permitted to drive for 24 hours following the surgery.
While Dr. Esmailian recommends waiting a full week before returning to work, some patients are able to do it in 5 or 6 days, provided the work does not require physical strain. The first week is also accompanied by a special supportive bra, and after this time it may be replaced by a sports bra. About two weeks after your breast implant surgery with Orange County plastic surgeon Dr. Esmailian, you can resume wearing your normal bras and bikini tops.
EXERCISE AFTER BREAST AUGMENTATION AT OUR ORANGE COUNTY OFFICE
Exercise and any sort of strenuous activity are prohibited for the first five days following surgery. Light exercise may be resumed at this point, but more vigorous physical activity should not take place until three weeks after surgery.

THE BREAST AUGMENTATION CONSULTATION: INITIAL EXAMINATION

1. Is breastfeeding possible following a breast augmentation?
Yes, an augmentation will not affect your ability to breastfeed; however, women should wait at least 6 months after their breast augmentation procedure to become pregnant. The incision type, periareolar vs inframammary, usually does not make a difference in a woman’s ability to be able to breast feed after this procedure.
2. Will my breast and/or nipple sensation change after a breast augmentation?
Typically not and the percentages are likely less than 5%. Those women who do experience changes may have complete return of sensation but at times this may take 1 year. Incision type, periareolar vs inframammary, does not change the risk of loss of nipple sensation. Large implant does increase the risk of nipple/areola loss of sensation.
3. How long does it take to see the results of a breast augmentation?
The good news is that the results of your breast augmentation will be visible immediately following your procedure. However, it does take some time for the breasts to take shape and form. It will generally take 4 months before the implants have settled and the final result is achieved.
SOME WOMEN CHOOSE TO HAVE A BREAST AUGMENTATION AND A LIFT
A breast augmentation at our surgical center in Orange County, in and of itself, cannot address severely drooping breasts; some women choose to have an augmentation to create fuller breasts and a breast lift to raise drooping breasts. Breast augmentation and a breast lift may be performed safely as a combination surgery, meaning can be performed at the same surgery. In very small percentage of cases, it may not be safe to perform both those procedures at the same time, therefore, the breast lift may be performed first and then breast implants place approximately 4 months later.

HOW TO CHOOSE YOUR BREAST IMPLANT SIZE
One of the first questions that many patients ask Dr. Farbod Esmailian at his Orange County office is: “What size implant should I choose?” If you want natural-looking results, you have to balance the reasons you want to have a breast augmentation with remaining somewhat conservative. When considering a breast implant size, think shape and proportion as opposed to cup size.
CONSIDER SHAPE AND PROPORTION
The cup size for breast augmentation patients is subjective because bra manufacturer sizes and the way bras are worn vary. Another issue that causes confusion is cleavage. Many patients believe that increasing their breast size will give them more cleavage. However, the amount of cleavage you have is determined by the width and shape of your chest as well as by how close your breasts are to one another.
HOW A BREAST AUGMENTATION AFFECTS CLEAVAGE
If you have a narrow chest and your breasts are close together, even a small implant can give you great cleavage. On the other hand, if you have a wide chest and there is a substantial space between your breasts, you will enjoy fuller breasts, but additional cleavage is not more difficult to obtain. In certain cases additional shaping can be performed and an appropriate implant diameter can be chosen to help with the cleavage area. When choosing your breast implant size, Dr. Esmailian will need to take into account your height, body frame, weight, hip width, current breast volume and shoulder width into consideration. Dr. Esmailian will take additional measurements during your consultation at his office in Orange County to help you choose the breast implant size that will provide you with the best results. He will measure your chest, nipple-areola complex (its size and position on the breast), the level of your breast droop (if any) and your natural breast size. BREAST IMPLANT SIZE VS. CUP SIZE – WHAT IS THE DIFFERENCE?
While some believe that the volume of an implant is comparable to a particular cup size, this is a misconception. An implant size is based on the amount of gel or fluid within the implant itself. The amount of gel or fluid inside an implant is measured in cubic centimeters (cc).
Keep in mind that as implant volume increases, a larger implant may be necessary to maintain proportion. Therefore, an individual who is 5’1” tall and petite needs a smaller implant size than an individual with broad hips and shoulders who stands 5’8” tall.
USE THESE STEPS TO DETERMINE THE BREAST SIZE YOU LIKE
STEP ONE: PICTURE COMPARISON
Look at the before and after breast augmentation photos on Dr. Esmailian’s website: Find a before photo of a patient who has breast characteristics and physical traits that are similar to yours. Although the implant size that you desire cannot actually be determined by viewing photos of someone else, you can find a photo that resembles the look you desire. This gives Dr. Esmailian insight into what you would like him to accomplish with your breast augmentation.
STEP TWO: THE RICE TEST
WHAT YOU WILL NEED:
- At least one pair of knee high nylons
- Dry rice
- Measuring cups (1 cup and 1/4 cup)
- A sports bra (without padding or an underwire)
- Various types of clothing (formal dress, cocktail dress, form-fitting t-shirts, favorite tops, etc.)
- A mirror that allows you to see your entire figure
You can ‘try on’ various breast sizes at home by performing the Rice Test. This process allows you to determine what looks too big and what seems too small for you. Although Dr. Esmailian does have various implant sizers that you can try on in his Orange County office, the Rice Test is a great starting point. In addition, the Rice Test is performed at home; therefore, you can take your time and ponder your options.
THE RICE TEST
1. Take a pair of knee high nylons (you should avoid using nylons that offer additional support as well as knee highs that have a reinforced toe) and fill each nylon with 1 ½ cups of rice.
2. Tie a knot on the top of each nylon – approximately 3 inches above the rice.
3. Shape the rice-filled nylons into patties. They must be as even as possible in projection and as large as your natural breast’s circumferential base.
4. Put the sports bra on and insert the pre-formed rice patties.
5. Try on the different outfits and tops you have gathered.
6. If you are not sure about a size, take a photo of yourself. After taking several photos of yourself in various size rice patties, you can compare them to one another. Ideally, take each photo wearing the same clothing item: This will make comparing the sizes easier.
CHANGING THE SIZE OF THE RICE PATTIES:
You can add or remove the rice by ¼ cup at a time and then try the clothing on again until you are happy with the way your breasts look.
Once you determine which size rice patties you think looks the best, bring them with you to your next appointment with Dr. Esmailian at his office in Orange County. He can use these to help determine your implant size. Understand that the “look” of the rice will not resemble your final breast shape but it does give a good indication for volume.
MENTOR VOLUME SIZING SYSTEM:
On your preoperative visit which is generally 2 weeks before the scheduled surgery, Dr. Esmailian will review the photos you bring in, as well as your measurement on the rice test as well as your measurements that were taken during your consultation. At this point, the Mentor Volume Sizing System is used to solidify your implant volume and to finalize the look you are striving for. Are Silicone Breast Implants safe?
Yes, the silicone breast implants we use today are safe. Technological advancements have led to rapid changes in silicone breast implant manufacturing, which has made these implants more stable than ever before. However, this question is understandable considering that in 1992 the FDA removed silicone implants from the market due to concerns related to their safety: Since that time, the ban was lifted because the concerns related to these implants were unfounded. Today, the silicone implants Dr. Esmailian uses are FDA-approved as effective and safe.
HOW DO I KNOW IF I AM A GOOD CANDIDATE FOR A BREAST AUGMENTATION?
If you are healthy, have breasts that look deflated, never fully developed or appear uneven, you are a good candidate for a breast augmentation with Dr. Esmailian. In addition, you should understand the benefits, risk factors and specifics related to recovering from breast implant surgery.
Can A Breast Augmentation In Orange County Fix Droopy Breasts?
Unfortunately, inserting breast implants cannot correct drooping breasts. However, Dr. Esmailian can perform a breast augmentation in conjunction with a breast lift (to address breasts that droop) during the same surgical session.
How Long Does It Take For The Incision To Heal?
The incision made during your breast augmentation will close within 48 hours. However, it takes from 4 to 6 weeks for the incision to heal completely.
How Long Will It Take For The Scars To Fade After Breast Implant Surgery?
Post-surgical scarring typically depends on the location of the incision and the technique that is used. While each patients’ results vary, scars usually reach their final appearance within 10 to 12 months of the procedure.
Will I Be In A Lot Of Pain After Surgery?
As with any surgery, you should expect to feel uncomfortable; however, most of Dr. Esmailian’s breast augmentation patients do not require any medications. A prescription for muscle relaxant and pain will be provided in case of need, as patient’s pain threshold vary.
WHEN WILL I BE ABLE TO RETURN TO WORK?
When you are able to return to work depends on your personal recovery as well as the type of work you do. What are the different incisions that are used and which one is the best?
There are four incisions that can be used for the placement of breast implants: transaxillary (armpit), transumbilical (thru the belly button), periareolar (bottom portion of the areola) and inframammary (in the tuck). The transaxillary approach and the transumbilical or TUBA approach have both been found to have a higher complication rate, especially revision rate. The two most commonly used approaches are the periareolar and the inframammary or IMF incisions. Both provide the surgeon very good visualization to perform the surgery accurately. Each has their advantages and disadvantages. The periareolar incision has a tendency to hide the incision better, especially in those patients with darker skin type who are more prone to developing hyperpigmentation (darkening) of their scars. The disadvantage with this approach is that we are finding out that is has a higher rate of capsular contracture. The inframammary incision in most patients is the best incision because it can hide well in the tuck and it has the lower rate of capsular contracture as it does not involve cutting thru any breast tissue.
What is better behind or in front of the muscle?
Implants placed behind the muscle are referred to as submuscular or subpectoral and implants placed in front of the muscle are referred to as sub-glandular. The placement of subglandular implants was very popular in the 1980s. However, we have realized that the one advantage of having implants in front of the muscle, the natural appearance, is heavily outweighed by the negative issues that arise. These issues include: capsular contracture, rippling, implant palpability, premature stretch and aging of the breast tissue and skin. Therefore, the standard now is to place the implants in a subpectoral position to avoid all the issues that come along with subglandular implants. The implant is not completely placed behind the muscle as the lower part of the muscle is released, dual plane. This results in the implant behind partially subglandular, in the lower 1/3, and therefore utilizing the benefit of the subglandular implant (having the natural look) with all the benefits of having the implants behind the muscle. There is one drawback of subpectoral breast implants and that is animation deformity. This is a temporary phenomenon that occurs when the pectoralis muscle is flexed. This will cause the implant to move/displace while the muscle is contracted and will return to position after the muscle is no longer contracted. This one drawback is minimal and is heavily outweighed with all the benefits.
What are the different profiles and which would work best for me?
Every manufacturer has their own nomenclature for the implant profiles. A profile of implant determines how much projection is obtained but also it determines how wide or narrow the implant is. Let us take the arbitrary implant size of 400 cc. A 400 cc implant in a high profile will be narrower than a moderate plus profile implant; however the high profile implant will be more projecting. The projection will determine how much fullness the patient will have in the upper part of the breast (upper pole). Again depending on the implant company, they will name their implants differently, for example: Extra full = Ultra High, Full = High, Moderate = Moderate Plus and so forth.
What is a dual plane breast augmentation?
This is the term that is commonly used when the implants are placed behind the muscle after the lower attachments of the muscle are released. This allows the implant to naturally sit along the breast crease to provide a very natural appearance but still provide the benefits that go along with placing the implants behind the muscle. The upper 2/3 of the implant is still positioned behind the muscle.
What are the types of breast implants available?
Silicone gel and gummy are exactly the same implant. Gummy is a term that is used to describe the silicone breast implants that are currently on the market. All the silicone breast implants that are available now are considered gummy. There are some implants that are gummier than others which I will describe later on. Saline and silicone breast implants are the same when it comes to the material that is used on the outside: both have a silicone shell. It is what is on the inside that is different: saline has physiologic solution (salt water) and silicone has a cohesive silicone gel.
What is the IDEAL breast implant?
The Ideal breast implant is a saline breast implant than is supposed to bridge the gap between the traditional saline implants and silicone implants. It decreases the rippling that can happen with saline implants and “sloshing”. The Ideal implant has a silicone shell like the traditional saline and silicone gel implants but it differs on the inside. There are multiple baffles that help slow down the movement of the saline and also give the outside shell some support and integrity. The Ideal breast implant a nice alternative to the traditional saline implant and helps improve the feel. However, I would not consider the Ideal breast implant an alternative to silicone gel implants.
What is a gummy implant or which implant is the gummiest?
Different implant companies now offer different cohesive levels in their silicone implants. This means that as a surgeon I can really fine tune the look and feel that a patient desires. For Allergan implants they offer Type 1: Responsive, Type 2: Soft Touch and Type 3: Cohesive (highly). As you go up from Type 1 to Type 3 the silicone get become more firm and has less settling. This also means that the shell will display less rippling as well at the expensive of making the breast fell a bit more firm. Mentor has their regular line but also has added the Xtra line. The gels in the two implants are the SAME, but they just have added more gel per shell to give more projection. Sientra has two different gel cohesive levels named: HSC and HSC+. The HSC is their regular line and the HSC+ is a more cohesive gel.
How big can I go?
Sizing is obviously an important aspect in deciding what works best and determing the look. We perform measurement, evaluate the skin envelope elasticity, look at photos, look at what the patient came up with the rice test, look at diagrams and finally use the Allergan/Mentor sizing system to size the patient in the office. We obviously listen and discuss exactly what the patient wants and doesn’t want so that we can deliver the expected results.
Your best path to determine what size would work and how big you can be would be to seek out a board certified plastic surgeon to discuss your options. Make certain the surgeon you choose has great deal of experience in breast implant surgeries.
How long is the surgery?
A breast augmentation procedure without a lift generally will take 1 hour. The procedure is performed at an outpatient surgery center. It is done under general anesthesia and will require about 45 minutes in the recovery room before the patient is allowed to go home.
How much pain will I be in after the surgery?
If performed meticulously, a breast augmentation procedure should not be very painful at all. I encourage my patients to go to dinner the night of their surgery. We prescribe muscle relaxants which help relax the pectoralis muscle which helps ease the discomfort. Some patients will choose to also take over the counter Tylenol to help with the discomfort. Patients describe the feeling as tightness and feeling sore.
What about recovery time, how much time do I need off of work?
Most patients return to work within 3-5 days. If your job requires physical activity then it may be a bit longer.
How long before I can exercise?
Patients are allowed to return back to full activity when they are 4 weeks out. At that point, all the restrictions would be removed. Patients are allowed to go back to the gym or any other exercise routine that they performed prior to their surgery.
Do I need to replace the implants every 10 years?
This is a very common question and the short answer is no. If you have saline implants, there is no kind of follow up needed. If the implant needs to be replaced you will know. This is because if there is a break or tear in the shell the saline (salt water) will leak out of the implant and be absorbed by the body. – These implants are slightly firmer than the Responsive gel implants, but not as firm as the gummy bear are. Silicone implants do require a slightly larger incision than saline implants do; however, in the event of a rupture, this thicker gel remains within the breast pocket.
SALINE IMPLANT CHARACTERISTICS
Saline implants consist of a silicone shell that is filled with a salt water solution. Since the implant’s shell is inserted empty, a smaller incision can be used. After shell insertion, Dr. Esmailian will fill the shell with salt water. A saline implant provides patients with a firmer, rounder breast. In the event of a rupture,the body will absorb the solution, which is harmless. However, the breast will be deflated and a replacement would be necessary sooner than later.
CHARACTERISTICS OF THE IDEAL IMPLANT®
This breast implant was designed by taking the best features of the saline and silicone implants, and combining them within a single silicone shell to create the IDEAL IMPLANT®. Although, IDEAL IMPLANT® breast implant is revolutionary, it is still not a replacement for silicone breast implants.
INCISION OPTIONS
There are four types of incisions used when placing breast implants. The incision Dr. Farbod Esmailian uses depends greatly on the type of implant being placed and, when there is more than one incision option, the patient’s preference.
Incision options include the:

Periareolar
– This incision is created along the lower portion of the areola. The areola is the darkened skin that surrounds each nipple. While creating this incision, Dr. Esmailian closely follows the outer edge of the areola. Precision is the key to providing the patient with the least visible scarring possible. For this reason, choosing an experienced, board-certified breast augmentation surgeon is critical. When the periareolar incision is used, the chance of loss of nipple sensation is no more than the inframammary incision.

Inframammary
– Dr. Farbod Esmailian creates this incision along the fold at the base of the breast. This incision location is ideal for women who want to significantly increase the size of their breasts using silicone breast implants. With the use of the Keller Implant Funnel, the incision used can be that same size as the periareolar incision. The resulting scar remains well hidden within the fold of the breast.

Axillary
– This incision is created in the armpit and is ideal for women who are having saline breast implants placed. Once the incision is made, the empty silicone breast-implant shell is placed and then filled with a salt water solution.

Periumbilical (Superior)
– This incision is rarely used and is only practical when placing saline breast implants. The periumbilical incision is created within the navel (belly button) and once the implant is inserted, it is placed above the pectoral muscle.
IMPLANT PLACEMENT
Dr. Farbod Esmailian will create a pocket in which to place the breast implant. This pocket can be created above the pectoral muscle or beneath it. The patient’s physical characteristics can affect which location Dr. Esmailian recommends.
Placement options:

Subglandular Implant Placement
– With this placement option, the pocket is created beneath the breast tissue, but above the pectoral muscle. With subglandular placement, patients enjoy a rounder, higher breast projection. In addition, when compared to its submuscular counterpart, patients experience less pain following their Orange County breast augmentation. This placement option is not meant for women who have very little natural breast tissue because lack of tissue increases the potential for implant visibility (e.g., implant edges and/or rippling).

Submuscular Implant Placement
– Dr. Farbod Esmailian creates the implant’s pocket beneath both the breast tissue and the pectoral muscle. This placement option provides the patient with a more natural-looking breast projection. In addition, submuscular placement decreases the likelihood of implant rippling and capsular contracture. Although this option may have a slightly longer recovery, the decrease in risks and issues outweigh placement of implants in the subglandular position.

– The anatomical implant is shaped like a teardrop. This option is ideal for women who have lost a considerable amount of volume in the lower portion of the breast.

– These implants are perfect for women who just want to add volume to the lower portion of their breasts as well as the upper portion.
IMPLANT SURFACE
Implant surface options:

Textured
– A textured implant’s surface feels similar to sandpaper. The textured surface helps the implant adhere to the tissue surrounding it. Once adhered, the implant will remain firmly in place. Texturing is mandatory with the use of anatomical implants.

Smooth
– An implant with a smooth surface can move more freely within its pocket than a textured implant can.
A BREAST AUGMENTATION USING HARVESTED AND REFINED FAT
Patients who are looking to increase the upper pole and perhaps a small increase the size of their breasts by no more than a cup are usually good candidates for a breast augmentation using their own harvested and refined fat instead of breast implants. With this procedure, Dr. Esmailian uses liposuction to harvest fat from the patient’s abdomen, flanks, thighs or bra line. He purifies the harvested fat and then strategically injects it into various locations within the patient’s breasts.

AN ORANGE COUNTY BREAST AUGMENTATION PROCEDURE WITH DR. FARBOD ESMAILIAN
This procedure usually takes one hour. Patients who are having multiple procedures should expect to remain within the surgical area longer.
Patients do receive general anesthesia; therefore, they are asleep throughout the entire procedure. Since they receive anesthesia, patients are not permitted to drive themselves home. For this reason, patients must arrive at the plastic surgery facility with a licensed driver.
Breast augmentation procedure steps:
1. Dr. Farbod Esmailian Visits the Patient in Pre-Op
Dr. Esmailian will visit the patient while she is still in the pre-op area. He will make various markings on her chest and breasts. These markings are used as a guide throughout the procedure. In addition, patients can ask any remaining questions at this time.
2. The Anesthesiologist Administers Anesthesia
Once the patient is sedated, the breast augmentation procedure can begin.
3. Creating the Incision
Dr. Farbod Esmailian will create the incision in the agreed-upon location.
4. Making the Implant’s Pocket
After creating the incision, Dr. Esmailian will make the pocket in which the implant will sit. The pocket needs to be slightly bigger than the implant is. This additional space allows the implant to move around a bit, which provides the patient with a result that looks and feels natural.
5. The Keller Funnel Procedure Method
When using this method, Dr. Farbod Esmailian inserts the breast implant into its pocket using a special device that is referred to as a Keller Funnel. By using this innovative device, the friction that occurs between the incision and the implant during placement is greatly reduced. In addition, the incision can be made smaller as well as less manipulation of the implant.
6. Closing the Breast Tissue and Incisions
Once Dr. Esmailian is satisfied with the location of the implant(s), he will close the breast tissue that leads to the pocket and the outer incision using specific suture techniques that are designed to minimize scarring.
7. Applying the Supportive Bra
The patient is supplied with a supportive bra. This support bra must be worn for the first week or two following a breast augmentation in Orange County, after that, most patients can begin wearing a loose fitting sports bra. Two weeks post-op most patients are able to wear their normal bras and swimsuit tops; however, patients should avoid push-up bras (such as those with wire) until cleared by Dr. Esmailian.
RECOVERING FROM AN ORANGE COUNTY BREAST AUGMENTATION
Directly after surgery, the patient remains in the recovery room until she is awake. Once she is fully aware, she receives information about caring for her incisions and the restrictions she must follow. Although some patients return to work on day five or six, Dr. Esmailian prefers that his patients wait 3-5 days before returning to work.
While the incision heals within 48 hours of the surgery, complete healing can take anywhere from four to six weeks. The scars typically fade substantially within a year of the surgery; however, it is essential that patients avoid subjecting their incisions/scars to sunlight or tanning beds as they are healing. Disregarding this precautionary recommendation could cause the scars to darken, permanently.
WHEN WILL I SEE RESULTS?
Although you will be able to see some results immediately, your true results will not be visible until the inflammation decreases and the implants become settled within their breast pockets. This typically takes about 4 months, sometimes longer.
HOW LONG DO BREAST AUGMENTATION RESULTS LAST?
The results of this procedure are achieved due to the breast implants, which should last at least 10 years. This does not mean that at the 10-year mark women with implants must have a revision surgery, unless a rupture occurs or some other type of problem arises, there is no need to replace the implants.
