TYPES OF MALE BREAST REDUCTION PROCEDURES AVAILABLE IN ORANGE COUNTY
LIPOSUCTION-ASSISTED GYNECOMASTIA SURGERY
A liposuction-assisted male breast reduction is an effective treatment for men who have excess fat, but do not have glandular tissue that needs to be removed.
A MINIMALLY INVASIVE MALE BREAST REDUCTION PROCEDURE
To perform this procedure, a tiny incision (approximately 5 mm) is created and then liposuction is performed.
Following liposuction, Dr. Esmailian makes an incision at the border of the areola and removes the glandular tissue from its overlying areola.
TWO-STAGE MALE BREAST REDUCTION FOR MEN WITH MODERATE-TO-SEVERE GYNECOMASTIA
The initial stage of this two-stage procedure involves removing excess fat through the use of liposuction. Once liposuction is complete, a Periareolar incision (an incision at the edge of the areola) is created. Through this incision, Dr. Esmailian removes additional fat, glandular and fibrous tissue.
The second stage of this procedure is performed once the blood supply is reestablished (generally 4 to 6 months later). Dr. Esmailian creates an incision around the patient’s nipple-areola complex. This stage of the procedure is used to reduce the size of the patient’s areola. For the majority of patients, there is no need for skin resection because adequate skin contraction is common.
A MALE BREAST REDUCTION IN ORANGE COUNTY – THE PROCEDURE
Dr. Esmailian’s patients receive anesthesia during their gynecomastia surgery. Since this is an outpatient procedure, all patients are required to bring a licensed driver with them on the day of their procedure.
1. PREOPERATIVE MARKING
Directly before surgery, Dr. Esmailian will make markings on the patient’s breasts. He uses these markings as a guide throughout the surgery.
Patients receive either local anesthesia with sedation or general anesthesia. General anesthesia is designed to put the patient into a deep sleep; whereas, local anesthesia with sedation puts the patient into what is referred to as a ‘twilight’ sleep.
3. THE LIPOSUCTION INCISIONS ARE CREATED
If liposuction is being performed, a small incision will be created on each side of the patient’s chest. The incisions may be located within the patient’s armpits or along a portion of his areola.
4. FAT IS LIQUEFIED AND REMOVED
The liposuction incisions are used to insert a thin, hollow tube. Dr. Esmailian introduces a tumescent solution (this special solution helps liquefy the excess fat) to the areas being treated. Once the tumescent solution has infiltrated these areas, the thin, hollow tube is gently moved back and forth. While moving the tube back and forth, a liposuction vacuum removes the excess fat via the thin, hollow tube (cannula).
5. SURGICAL DISSECTION FOLLOWING LIPOSUCTION
Dr. Esmailian may be able to remove excess glandular tissue at this point. As he is removing the excess glandular tissue and fat, he will sculpt the patient a new, natural-looking chest contour.
6. SEVERE GYNECOMASTIA REQUIRES ADDITIONAL INCISIONS
Males who have severe gynecomastia and/or sagging, stretched skin around their breasts may need surgical tissue excision to attain the results they desire. Through tissue excisions, Dr. Esmailian can remove a greater amount of skin and/or glandular tissue.
Incision length and location depends on the amount of skin, and glandular tissue removal the patient needs to achieve his goal. However, in general, incisions are located within the natural creases of the patient’s chest or around the edge of the patient’s areola. Dr. Esmailian concentrates on placing incisions within areas that will hide the resulting scars well.
7. INCISIONS ARE CLOSED
Once the excess glandular tissue and/or excess skin is removed, Dr. Esmailian will close the incisions using absorbable sutures, a skin adhesive and/or surgical tape.
8. THE COMPRESSION GARMENT IS APPLIED
To minimize inflammation and offer support to the tissues as they heal, patients generally receive a compression garment. This garment needs to be worn for several weeks following gynecomastia surgery.

RECOVERING FROM GYNECOMASTIA SURGERY IN ORANGE COUNTY
- Following gynecomastia surgery, males should expect to experience some inflammation. Pain is minimal; however, patients will feel sore for a few days. Dr. Esmailian may prescribe pain medication or recommend over-the-counter medications.
